På den tiden fick reumatiker inte träna hårt, något Christina Opava intuitivt kände var fel. Idag är hon professor i fysioterapi och en ledande forskare inom fysioterapi för personer med reumatism. Hon anser att forskningen och behandlingen måste bli bättre på att mäta individuella framsteg. Jag är gift och mamma till två söner. Att umgås med mina vänner som bor runt om i Sverige är riktigt härligt. Jag älskar att cykla och gör det som motionsform, eftersom det är obekvämt att gå längre sträckor eller springa. Ingen dag är den andra lik. Utmaningen med mitt arbete är att hela tiden stå på topp och aldrig stagnera. Som forskare är man inte bättre än sin sista vetenskapliga artikel eller senaste forskningsanslag. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Frustrationen för mig i jobbet är att jag är en “udda figur” i ett biomedicinskt sammanhang. Vi som fysioterapeuter bedöms utifrån måttstockar som ställts upp utifrån att människan är ett biologiskt ”system”, medan vi försöker studera människans kropp och psyke i ett socialt sammanhang, vilket blir mera komplext. Även om vi kan göra den sortens studier med den sortens utvärderingsmetoder som rankas högst i den biomedicinska forskningen, så är det inte alltid lämpligt. Så det vi då gör istället räknas inte lika mycket. Jag tycker att det är frustrerande att det fortfarande är så, efter alla år, att forskningen domineras av det biomedicinska perspektivet. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Här tror jag att de organiserade patienterna kan ha en viktig roll. Ett exempel var när Socialstyrelsen arbetar fram riktlinjer för reumatikervården. Då blev Reumatikerförbundets åsikt: “Vi vet att styrketräning är bra för oss”, ett viktigt skäl till att rekommendationen om det blev starkare, även om de studier som fanns inte gav 100 procent evidens med bästa kvalitet.\",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"heading-2\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Vad fokuserar du på i din forskning? \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Fysisk aktivitet och framför allt numera: Hur kan vi nå så många så möjligt och hur åstadkommer vi beteendeförändring på sikt, så att deltagarna inte enbart tränar under en sex veckors forskningsstudie. Vi använder oss därför av beteendemedicinska tekniker, att sätta mål och utvärdera.\",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Exempel på beteendemål: Jag ska träna så här många gånger i veckan. Jag ska göra det klockan si eller så. Jag ska förbereda mig såhär, packa väskan och ta med till jobbet. Jag ska äta ordentligt vid fyratiden för att orka gå iväg efter jobbet. När målen har satts så utvärderar man om det blev det som man tänkt. Blev det inte det så reflekterar man över varför och vad som krävs för att målen ska uppfyllas framöver. Om man uppfyllt målen är det viktigt att belöna sig själv. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"heading-2\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Vad forskar du om idag?  \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"PARA-projektet, som har drivits under nästan tio år, innehåller väldigt många olika delar. Ett resultat från det senaste året visar att vårt fysiska aktivitetsprogram ledde till minskad hjärtsvikt. Det kanske till och med kan minska risken för att få hjärtsvikt. Det är häftigt. Sedan tidigare vet vi att styrketräning och konditionsträning av hög intensitet ger den effekten. I PARA-projektet har vi istället visat att promenad och cirkelträning också ger denna effekt. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Det som är svårt att visa på gruppnivå är just de specifika individuella vinsterna. En person kan äntligen leka med sina barnbarn, en annan kan slippa genomgå en knäoperation och bli så bra att den klarar av att gå uppför en skidbacke och en tredje kan sätta sig ner och resa sig upp istället för att sitta stilla större delen av tiden. De här resultaten slår inte igenom på gruppnivå men för individen är det livsviktigt. Vid den tiden praktiserade jag på reumatologen, av en ren händelse. Jag cyklade till sjukhuset och sjukgymnastiken låg i källaren och jag sprang ner för trapporna. Där träffade jag många patienter som inte fick träna genom att spänna sina muskler och röra på lederna samtidigt. “Va, ska detta gälla för mig också?”, tänkte jag. Det lät konstigt. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\" Som exjobb gjorde jag en dynamisk träningsstudie med en träning som heter PNF. Sjukgymnasten lägger på motstånd medan patienten jobbar i diagonala armrörelser. Vi mätte handkraften på våra patienter före och efter träningen och såg att handstyrkan ökade. Jag kunde inte acceptera det rådande synsättet att reumatiker skulle vila istället för att röra på sig. Jag ville ändra praxis. Det är det som har drivit mig. Jag har också hela tiden inte bara varit intresserad av vad som händer i kroppen utan vad som händer med hela människan som tränar.\",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"En av de saker som har varit det mest problematiska med min egen sjukdom var att jag som ensamstående mamma hade väldigt svårt att fysiskt nå fram till mina tonårssöner. Jag kunde inte spela boll eller skojbrottas, till exempel. Sådana saker framgår inte i några av vårdens frågeformulär. För mig var det här värre än själva smärtan i sig, värre än tröttheten. Jag kunde inte nå barnen i de här aktiviteterna och skapa den typen av relation. Såna aspekter behöver beskrivas i kvalitativa studier. Det kan vara specifikt både fysiskt och ännu mer komplext när vi kommer till det psykosociala. Det är svårt att fånga hur var och en mår med de grova mått som används nu, till exempel i register. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"heading-2\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Vad är din personliga relation till fysisk aktivitet? \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Periodvis har jag gått på olika gym. Cirkelträning, tre varv med tio stationer på 30 minuter, passar mig bra. Det är också viktigt att gymmet ligger nära mitt hem. Min plan nu när jag snart går i pension är att ta mer hand om mig själv. Äta bra och regelbundet och cykla för motionens skull. Jag vet hur svårt det kan vara att få till det här med träning. Jag tycker faktiskt inte att det är så roligt att lyfta vikter eller konditionsträna. Det tar också tid från annat. Alla vi som automatiskt inte älskar träning kämpar och jag också! Jag ska också försöka komma igång med någon typ av dans eftersom jag älskar det. Det viktiga är ju att hitta något som jag gillar och därför väljer att fortsätta med länge.\",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"heading-2\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Hur har du hanterat din RA?\",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Mina viktigaste strategier för att klara av min RA är att hålla mig sysselsatt med saker jag tycker är roliga och där har jobbet varit både viktigt och meningsfullt. Läkemedelsbehandling har också varit viktig under alla år.\",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Jag har inte hållit min RA hemlig men inte heller pratat om det jättemycket. Barnen och de flesta av mina vänner säger att de aldrig tänkt på mig som sjuk. Möjligtvis har de kanske periodvis tänkt på mig som lite lat, att jag har legat på soffan en del. Min väninna sa att det var så mysigt att komma och hälsa på mig för då kunde man ligga och mysa i soffan hela eftermiddagen eller att det var så skönt att promenera med mig för att man ser så mycket, eftersom jag gick långsamt. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Jag har verkligen upplevt den kroniska tröttheten men när jag för snart 20 år sedan fick ett nytt läkemedel så försvann den. Mentalt har jag alltid varit alert men helt plötsligt kunde jag också vara alert fysiskt. Det var så skönt att få vara mig själv, få den energi jag glömt att jag hade och visa alla att jag kanske inte är så lat ändå. Det var häftigt att den kom tillbaka. Även om jag fortfarande kan bli så där förlamande trött är det stor skillnad jämfört med hur det var innan. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"heading-2\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Vad ser du i framtidens forskning och fysisk aktivitet och RA? \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Jag drömmer om att det ska finnas träningscenter ute i samhället där fysioterapeuter inom hälso- och sjukvården kan förlägga en del av sin arbetstid och erbjuda människor med kroniska sjukdomar sin kompetens i ett ”friskare” sammanhang. PARA-projektet var ett steg på den vägen men än så länge finns det för stora strukturella hinder för att genomföra detta. Det behövs ett förändrat synsätt och andra strukturer, till exempel ersättningssystem, för att det ska kunna bli verklighet. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Jag tror inte att det är så enkelt som att alla reumatiker “kan sköta sin träning själv”. Många kan det, men det kanske inte är de personer som bäst behöver träna, utan det är nog de redan frälsta många gånger. Har man aldrig tränat, aldrig varit på ett gym och så börjar man få ont och kanske blir lite svullen någonstans eller drar på sig övervikt, då är det inte så lätt att bara börja. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Jag tycker att FaR (Fysisk Aktivitet på Recept)* är ett jättebra stöd. Det kan hjälpa vissa över tröskeln och då behöver det vara tydligt skrivet och väldigt specifikt. Kanske skulle läkare och fysioterapeuter prata om FaR mer. Oftast är det de som frågar efter FaR som får det men många vet nog fortfarande inte att det finns. Ett FaR reducerar kostnaden för patienten. Det kan göra det lite lättare för patienten att testa. Främst gör Fysisk Aktivitet på Recept att patientens förståelse ökar för hur viktigt fysisk aktivitet är. Någon som du kan ringa och komma på återbesök hos om det inte blev som du planerat. \",\"marks\":[],\"data\":{}}],\"data\":{}},{\"nodeType\":\"paragraph\",\"content\":[{\"nodeType\":\"text\",\"value\":\"Vi har ju våra hälsofrämjande riktlinjer, 150 minuters aktivitet i veckan plus styrketräning två gånger men allting är bättre än ingenting. Bara att släpa sig ut och gå runt kvarteret, gör det en vecka så kanske du kan göra två kvarter veckan därpå. Man ska inte tänka att “kan jag inte följa riktlinjerna så är det ingen idé att jag gör någonting”. Jag vet också att många önskar sig direkt tillgång till reumatolog och andra hälsoprofessioner för rådgivning via till exempel en chatt.
